Job Description:

Under the supervision of the Vice President – Clinical Services, the Nurse Practitioner (NP) will provider direct patient care to AZPC members to improve outcomes in quality, patient experience, and help mitigate utilization of resources in support of AZPC organizational goals and objectives. The NP shall perform autonomously with an emphasis on disease prevention and health management.
POSITION DUTIES & RESPONSIBILITES
- Performs quality patient care within NP score of practice and ensures delivery of effective and appropriate care consistent with established regulatory, licensing, and evidence-based practice standards.
- Performs patient annual wellness visits in a variety of settings to include, but not be limited to, the patient's private residence, assisted living facility, and primary care provider office, specialist office, hospital, or skilled nursing facility.
- Conducts chard review to assist in supporting proper diagnoses, status and plan of care specific to patient acute and chronic conditions.
- Assesses patient for all acute and chronic conditions and diagnoses each condition that impacts the patient's care and treatment.
- Adheres to documentation standards and completes documentation of assessed acute and chronic conditions with the following documentation components within timeframe outlined by AZPC guidelines
- Correct diagnosis code
- Status of condition
- Provider plan to address each diagnosis code
EDUCATION, TRAINING AND EXPERIENCE
- Master's Degree in Nursing
- Board Certification as Advanced Practice Nurse from a nationally recognized certifying body as required by the Arizona State Board of Nursing.
- Family Nurse Practitioner or Adult Nurse Practitioner required. Geriatric experience preferred.
- Current unrestricted State of Arizona RN License.
- Current Arizona Advanced Practice Nursing license; temporary license will be accepted until receipt of permanent licensure pending national board certification.
- Full unrestricted prescriptive and dispensing privileges with DEA licensure required.
- Current basic CPR certification or ability to obtain within 30 days or hire.
- Flexibility in their work environment to travel throughout the valley is required.
- Self-directed and must have excellent organizational skills.
- Excellent interpersonal communication (written and verbal) skills.
- Current Arizona driver's license and proof of insurance.
